Email is currently used for appointments or other general information
only, and not read daily. Email is not opened by a doctor or nurse and
no medical advice will be given on line. If you have a medical emergency,
call the office or call 911.
Insurance Information:
We participate with Aetna, Blue Cross, Cigna, Healthkeepers,
Medicare, Medicaid, Carenet, Southern Health, Travelers and other commercial
insurance companies.
If you participate in an HMO, you are responsible for obtaining your
referral in advance. If you do not have a necessary referral, you may
pay for the visit yourself or you may reschedule. Please understand
that we are not responsible for obtaining your referral, nor can we
make other patients wait because your referral is late. We are happy
to submit bills directly to insurance companies. To reduce the cost
of billing, co-payments are collected at the time of the service.
We accept cash, personal checks, visa and master card.
Nurse Practitioners
Because of the large number of patients waiting to see Dr. Armstrong,
most new patients are seen first by a nurse practitioner who specializes
in ear, nose and throat disorders. She will participate with Dr. Armstrong
throughout your health care in his office, and be available to you even
when he is at surgery. She is an excellent resource for education regarding
your condition and treatment, and will help to maintain a more personal

New Patient Registration
If you are a new patient to this practice, we will be grateful if you
can arrive approximately 15 minutes early for your appointment. This
will give time for the receptionist to review your insurance documentation
and enter you into our computer database. It will save us time if you
can carefully complete the patient information form. It is very important
that you include accurate insurance information and the name of your
referring and primary care physicians.
Please also accurately list any medical problems, medications, dosages
and medical allergies. By carefully completing this
form you will minimize the risk of having a dangerous drug interaction
as a result of any treatment that your doctor might prescribe. It
is also important that you bring with you your insurance card, as well
as some type of identification, such a driver's license.
These forms may be completed on-line and then printed,
or printed first and completed by hand. You may E-mail the form
to us in advance, but please be sure to bring the form with you for
