New Patients
If you are a new patient to this practice:

  • Please arrive approximately 15 minutes early for your appointment. This will allow time for the receptionist to review your insurance documentation and enter you into our computer database.
  • Include accurate insurance information and the name of your referring and primary care physicians.
  • Accurately list any medical problems, medications, dosages and medical allergies. By carefully completing this form you will minimize the risk of having a dangerous drug interaction as a result of any treatment that your doctor might prescribe.
  • Please bring any relevant xrays, sleep studies, or medical records with you.
  • It is also important that you bring your insurance card, as well as some type of identification, such a driver's license.

Registration forms need to be completed online and we will print them in our office. All communication needs to be conducted via our secure web portal to ensure maximum security.

Nurse Practitioner Appointments

Because of the large number of patients waiting to see our doctors, most new patients are seen first by one of our nurse practitioners who specializes in ear, nose and throat disorders. They will participate with Dr. Armstrong throughout your health care in his office, and be available to you even when he is at surgery. They are an excellent resource for education regarding your condition and treatment, and will help to maintain a more personal approach to your care.
